The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

The responsibilities of the Clinic RN include, but are not limited to:

Responsibilities:

  • Conducts initial and quarterly assessments of each assigned participant’s health and social status, identifies overt problems/needs, sets priorities, addresses problems requiring immediate referral and/or follow-up, and interprets and records the latest diagnostic results.
  • In coordination with the Interdisciplinary Team, develops a Plan of Care based on the nursing process that incorporates immediate and ongoing care needs, and includes the perspectives of the participant and their family.
  • Provides care that adheres to accepted practice standards and meets the age-related characteristics and needs of participants.
  • Delivers direct care that includes, but is not limited to, wound care, IV therapy, blood sugar checks, IM injections, vital sign checks, and follow-up on episodic problems previously identified by the Primary Care Physician (e.g., Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infections, or other health concerns).
  • Oversees the organization of the BCSC exam and treatment rooms and coordinates with other Interdisciplinary Team members to schedule participants in a way that ensures day-to-day operational efficiency and participant well-being.
  • When assigned, participates in daily morning meetings to update other Interdisciplinary Team members on changes in participants' conditions and transitions between home, supportive housing, nursing homes, hospitals, and other residential settings. Provides briefings to the Interdisciplinary Team on participant health issues, daily schedules, planned tests, appointments, and scheduled medical specialty visits.
  • Records participant assessments, reassessments, and care provided in the medical record.
  • Consults with and educates the participant, family members, and/or caregivers regarding the disease process, self-care techniques, prevention strategies, and medical interventions.
  • Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.
  • Ensures that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician have been completed.
  • Coordinates all medication management for BCSC, including stocking non-prescription medications, ordering prescription medications, administering medication in the PACE Center, and monitoring participant compliance. Arranges for the delivery of needed medications and supplies to participants' homes.
  • Collaborates with the Primary Care Physician to ensure protocols and standing orders are followed for bowel care, foot care, skin care, urinary tract infections, vital signs, weight management, and wound care. Provides continence training to participants as needed.
  • Prior to the use of physical or chemical restraints, conducts an assessment, documents that restraint alternatives have been attempted, and obtains a physician’s order for restraints. Obtains family consent or refusal for the use of restraints. Monitors and documents that the participant has a specific medical symptom that cannot be addressed by a less restrictive intervention, and that restraint is required to treat the symptom, protect the participant’s safety, and help the participant achieve their highest level of physical or psychological well-being.
  • Supports the Infection Control function in coordination with the Quality Assurance Coordinator.
  • Actively participates in the Quality Assessment and Performance Improvement (QAPI) process.
  • Recognizes inappropriate participant care management or service delivery by other Interdisciplinary Team members, and addresses these concerns directly or refers them to the RN Clinic Supervisor immediately.
  • Identifies emergency situations and takes appropriate action, including coordinating direct hospital admissions from the clinic to the hospital as necessary.
  • Understands and demonstrates respect for participant rights, and refers ethical issues to the BCSC Ethics Committee.
  • Coordinates with the Pharmacist to support participant care.
  • Coordinates with the Home Care Coordinator to support participant care at home.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors, and others.
  • Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
  • Understands the importance of community involvement and participates, as appropriate, in activities that connect BCSC to its communities.
  • Demonstrates proficiency in delivering age-specific care.


Experience and Skills:

  • Degree and Diplomas: Graduate of a school of professional nursing.
  • Certificates and Licenses: RN currently licensed by the California Board of Registered Nursing. 
  • Verification of completion of CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a health care provider is required for employment.
  • Experience: One year in acute care setting, with at least one year caring for the frail or elderly. 
  • A Public Health Nursing background and/or experience with PACE (Program of All-inclusive Care for the Elderly) is preferred.
  • Bilingual in Spanish is preferred.
