The Document Control Clerk is responsible for organizing, managing, and maintaining patient records and company documents in compliance with legal, regulatory, and operational standards. This role supports the clinical and administrative teams by ensuring documentation is accurate, up-to-date, and easily accessible.
Maintain and update patient records in accordance with HIPAA regulations and company policies
Scan, file, and archive documents both electronically and physically
Ensure all documents are labeled, organized, and stored appropriately for easy retrieval
Monitor and track the flow of incoming and outgoing documentation
Assist in the preparation of reports, forms, and patient files as needed
Perform routine audits of files and documents to ensure accuracy and completeness
Collaborate with administrative and clinical staff to resolve documentation issues
Manage document retention schedules and handle document disposal per policy
Support front desk or administrative tasks as required
High school diploma or equivalent (required); additional administrative or records management training is a plus
1+ years of experience in document control, records management, or administrative support (healthcare experience preferred)
Strong attention to detail and organizational skills
Proficient in Microsoft Office Suite (Word, Excel, Outlook)
Experience with electronic medical records (EMR) systems is an asset
Ability to handle confidential information with discretion
Excellent communication and time-management skills
Office environment within a physical therapy clinic
May require prolonged sitting, standing, or walking
Some lifting of files or office supplies may be required
