Job Title: Home Visit Nurse Practitioner

We are seeking experienced Nurse Practitioners to join our team in Manhattan, NY as Home Visit Nurse Practitioners. In this role, you will provide comprehensive, patient-centered primary care to individuals in the comfort of their own homes.
Responsibilities - Conduct comprehensive health assessments for patients in their homes
- Develop individualized care plans based on assessments and patient needs
- Provide ongoing medical management and coordination of care
- Educate patients and their families on healthcare management and preventive care
- Collaborate with other healthcare providers to ensure continuity of care
- Document patient encounters and maintain accurate medical records
- Adhere to all regulatory and compliance requirements
Qualifications - Master's degree in nursing from an accredited program
- Current and unrestricted Nurse Practitioner license in the state of New York
- Certification as a Nurse Practitioner in Adult or Family Health
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in a primary care setting
- Excellent clinical skills and ability to work independently
- Strong communication and interpersonal skills
- Ability to travel to patient's homes throughout Manhattan
